Ë¿¹Ï¶ÌÊÓƵ Human Services Organization Support Challenge
In 2021, we launched a new initiative to recognize organizations and programs that make significant impacts on communities’ toughest health issues. Through the Ë¿¹Ï¶ÌÊÓƵ Human Services Organizations Support Challenge, we hope to inspire innovation on reducing health disparities. This includes moving the needle on health equity, especially for those with special care needs, both health and social.
The states vary from year-to-year, and the focus varies by state.
North Carolina
According to the (CDC) in the last decade, there has been an increasing awareness of the importance of addressing the social determinants of health (SDOH), the non-medical conditions that influence health outcomes, as a systematic strategy for improving health, particularly among groups that are disproportionally affected by SDOH. While health care is important, it is estimated that these conditions, ranging from structural racism to socioeconomic factors, drive as much as 50% of health outcomes.
Through the Ë¿¹Ï¶ÌÊÓƵ Human Services Organization Support Challenge, the Ë¿¹Ï¶ÌÊÓƵ Foundation would like to support those human services organizations that are focused on the social determinants of health and the overall whole-person health.
Applicants should be organizations or programs that focus on these areas:
- Whole-person Health
- Social Determinants of Health (SDOH)
- Behavioral Health
- Maternal and Child Health
ELIGIBITLY CRITERIA
Organizations must be a 501(c)(3) tax-exempt nonprofit entity, according to the Internal Revenue Code.
All questions on the nomination form need to be filled out. Incomplete nominations will not be reviewed. A nomination may be rejected if it does not sufficiently and specifically describe the activities and impact or, in the Ë¿¹Ï¶ÌÊÓƵ Foundation’s discretion, does not align with the criteria outlined on our website.
All nominations need to be completed electronically using the link below. Nominations submitted via email will not be accepted.
The following programs are not eligible to be nominated:
- Reimbursable Services
- Capital Projects
- Feasibility Studies
- Marketing or Awareness Campaigns
- Political or Election-related Activities
- Research Studies or Projects
- Promotional, Social or Community Events
- Individuals
- For-profit Enterprises or Activities
- Organizations established for less than 2 years
- Organizations managed by a Ë¿¹Ï¶ÌÊÓƵ employee or board member
- Organizations managed by a family member of a Ë¿¹Ï¶ÌÊÓƵ employee or board member
SELECTION PROCESS
All applications will be reviewed by a Ë¿¹Ï¶ÌÊÓƵ Review Committee, comprised of a cross-functional, multi-state panel of experts, and Ë¿¹Ï¶ÌÊÓƵ Foundation staff. The Ë¿¹Ï¶ÌÊÓƵ Review Committee will review all relevant information submitted to determine which of the applications are the most qualified recipients and meet the criteria.

AWARDS
First place: $50,000 award (1 organization)
Second place: $20,000 award (3 organizations)
Third place: $10,000 award (2 organizations)
Fourth place: $5,000 (2 organizations) Fifth place: $2,500 (2 organizations)
Selected organizations must sign Ë¿¹Ï¶ÌÊÓƵ’s award agreement letter, which includes an agreement to provide a short report on how the award is used for informational purposes.
