  • What kind of services are offered through Ë¿¹Ï¶ÌÊÓƵ Life Services® program?

    • One of the services offered is job and career help. You can get personal life coaching, help finding a job and rides to job training and interviews. Job coaches help you get ready for interviews and build a resume. They also help after you start a new job. Ë¿¹Ï¶ÌÊÓƵ JobConnect also gives you free GED testing and study help.

Ë¿¹Ï¶ÌÊÓƵ Rewards

  • What rewards does Ë¿¹Ï¶ÌÊÓƵ offer to members?

    • Ë¿¹Ï¶ÌÊÓƵ offers three rewards programs to HIP and HHW members:

      MyHealth Rewards for Adults – A health and wellness program that rewards adults age 18 and older. Some reward activities vary by age, gender and member health issues. 

      Babies First – A program that rewards pregnant women and their babies up to 15 months up to $240 for healthy activities. Note, pregnant women age 19 and older can also join the MyHealth adult rewards program. Pregnant women 18 year of age and under can also join the Kids First reward program.

      Kids First – A program that rewards children ages 16 months through 17 years. Rewards are based on each child’s age, health and wellness needs.

  • How does the MyHealth rewards program work?

    • As an eligible Ë¿¹Ï¶ÌÊÓƵ member age 18 and over, you can start earning MyHealth rewards when your benefits start. You can earn rewards for doing healthy activities, like taking your Health Needs Screening. Start by making a private online account through . After you log in, use the Health tab to get to MyHealth. There you can see what types of things earn you rewards, track your progress and collect your rewards.

      You can earn rewards by doing online activities or getting preventive screenings shown in your MyHealth account. There is no need to submit paperwork.

      You can choose to get gift cards to places like iTunes, Google Play and more.

Joining Ë¿¹Ï¶ÌÊÓƵ

  • Who is Ë¿¹Ï¶ÌÊÓƵ?

    • Ë¿¹Ï¶ÌÊÓƵ is a managed health care plan. We work with Indiana Family and Social Services Administration (FSSA) to serve Hoosier Healthwise (HHW) and Healthy Indiana Plan (HIP) members in Indiana.

      Hoosier Healthwise is a health care program for children up to age 19 and pregnant women that meet certain income requirements. The program covers at little or no cost medical care like:

      • Doctor visits
      • Prescription medicine
      • Mental health care
      • Dental care
      • Hospitalizations
      • Surgeries
      • Family planning

      The Healthy Indiana Plan (HIP) covers adults age 19-64 whose incomes are less than 138% of the federal poverty level and are not eligible for Medicare or another Medicaid Plan. HIP requires you to make a minimal monthly contribution to your coverage based on the amount of your income.  For more information about the Healthy Indiana Plan, visit the  website.

  • How can I become a member of Ë¿¹Ï¶ÌÊÓƵ?

    • The state of Indiana decides if you are eligible for Hoosier Healthwise (HHW) or Healthy Indiana Plan (HIP). For more information, review the eligibility information on the

  • What are the advantages of being a member of Ë¿¹Ï¶ÌÊÓƵ?

    • As a Ë¿¹Ï¶ÌÊÓƵ member you will enjoy all the same necessary medical services that are provided to anyone who is part of an Indiana Medicaid program. Some examples are preventive checkups, inpatient and outpatient hospitals services, and more. You will also get these additional Ë¿¹Ï¶ÌÊÓƵ services:

      • Ë¿¹Ï¶ÌÊÓƵ24®: Our 24/7/365 Nurse Advice Line
      • Ë¿¹Ï¶ÌÊÓƵ Job Connect™: Job assistance including coaching and training
      • Rewards for healthy activities including programs like Babies First® (rewards for prenatal and postnatal care), Kids First  and MyHealth. Details for these programs can be found on the Ë¿¹Ï¶ÌÊÓƵ services pages.
      • Easy access to health care when and where you need it with retail health clinics, urgent care centers and letting you connect with a doctor online
      • An online smoking cessation program through MyHealth
      • Text4baby: A free text message three times per week, timed to the due date or baby’s birth date, through pregnancy and up until the baby’s first birthday
      • Medication Therapy Management: One-on-one medication review and consultation with your pharmacist to ensure your medications are appropriate and safest for your healthcare needs
      • Ë¿¹Ï¶ÌÊÓƵ Re-Entry Program™ assists members who are returning citizens joining society after incarceration.
      • Driver’s License Reinstatement Program helps members reinstate their driver’s license in the event of a lapse, suspension or revocation.
      • Expungement Program: A fund designed to assist members with the expungement process.
      • myResources: An interactive tool to help connect members to local low-cost and no cost programs and services such as food, shelter, financial support and more.
    • What is My Ë¿¹Ï¶ÌÊÓƵ®?

Get a Ë¿¹Ï¶ÌÊÓƵ member ID Card, Plan Documents or Change Your Doctor

  • How do I get a new Ë¿¹Ï¶ÌÊÓƵ member ID card?

    • You can get a new member ID card by logging in to your account and clicking Request ID Card. You can also ask for an ID card from the Ë¿¹Ï¶ÌÊÓƵ mobile app or by calling Member Services at 1-844-607-2829 (TTY: 1-800-743-3333 or 711) Monday through Friday from 8 a.m. to 8 p.m. Eastern Time.
  • How do I get a new Ë¿¹Ï¶ÌÊÓƵ member handbook or provider directory?

    • Ë¿¹Ï¶ÌÊÓƵ sends a new member booklet to all new members. Your booklet contains a post card to order a printed member handbook and/or provider directory. Just tear out the cards and drop in the mail.

      You may also view the Member Handbook online. Or you may use the  search tool to see the most up-to-date list of providers in your area.

  • How do I select or change my primary care provider (PCP)?

    • You are not required to see the primary medical provider (PMP) listed on your card. If you want to change your PMP, you can log in to your  account to choose a new doctor. You can change your PMP to another health partner who participates with Ë¿¹Ï¶ÌÊÓƵ.

      Ë¿¹Ï¶ÌÊÓƵ will let you know that your PMP has changed and the date you can start seeing the new PMP. The Member Services department can also help you schedule your first appointment, if needed. For the names of the PMPs that serve Ë¿¹Ï¶ÌÊÓƵ members, please use the  tool.

  • What is My Ë¿¹Ï¶ÌÊÓƵ®?

  • What if I have a health question after hours?

    • If you have a health or medical question, you can call Ë¿¹Ï¶ÌÊÓƵ24®, our nurse advice line, at the number on your member ID card. Ë¿¹Ï¶ÌÊÓƵ24 is available 24 hours a day, 7 days a week, including holidays.

  • What if I have mental health or substance use concerns?

    • Call the Behavioral Health Crisis line at 1-833-227-3464 to speak to a licensed behavioral health professional. Or call the Addiction Hotline at 1-833-674-6437 (TTY: 1-800-743-3333 or 711).
  • How can I get help in another language or format?

    • Call Member Services at 1-844-607-2829 (TTY: 1-800-743-3333 or 711). We can get someone who can sign or speak your language to help you talk with us or your doctor. And it is free. We can also give you some printed materials in some other languages. Let us know. We can help.
