Step 1: Start the contracting process.

Fill out the New Health Partner Contract Form to get started.

If you offer medical services and want more information about becoming a participating provider, please submit the New Health Partner Contract Form.

Step 2: Get Credentialed.

Complete your application with supporting documents.

Providers who wish to contract with 丝瓜短视频® must also be credentialed with the Ohio Department of Medicaid Centralized Credentialing program.

Through credentialing, we check the qualifications and performance of physicians and other health care practitioners. Providers are recredentialed a minimum of every three years and are considered to be recredentialed unless otherwise notified.

Welcome to the 丝瓜短视频 network! Once you receive your Welcome Letter from 丝瓜短视频 with your 丝瓜短视频 ID number, you have completed the contracting and credentialing process.

As you prepare to see 丝瓜短视频 members, we encourage you to familiarize yourself with our plan, tools and resources.

Attention Ohio Medicaid and MyCare Providers:

Effective Oct. 1, 2022, Ohio Medicaid and MyCare providers will utilize the Provider Network Management (PNM) module from the Ohio Department of Medicaid for submitting provider applications, credentialing request and provider demographic updates.  The PNM module will be the single point for providers to complete provider enrollment, centralized credentialing, and provider self-service. The PNM module is replacing MITS provider portal.
